The same note mentions Symon Hoflant de Velsen from the diocese of Utrecht as the scribe. Wermboldus Overstege de Campis is mentioned as scribe on fols. 77. Dating of the other leaves based on the scribe and on the astronomic tables. The beginning of "Super libro IX Almansoris" is mutilated. "Super libro IX Almansoris" is a commentary on book IX of Rhazes' Liber Almansoris with an image of the Zodiac (fol. 1). Super libro IX Almansoris" "followed by "Adversaria medica" on fols. 63v-65v and a table of contents on fol. 66.
